Formerly called juvenile rheumatoid arthritis, juvenile idiopathic arthritis (JIA) is a type of arthritis that occurs from childhood until the teenage years. Signs and symptoms are similar to those of adult arthritic conditions, including joint pain or stiffness, rash, fever, tender or warm joints, and blurry vision. Doctors diagnose six types of JIA. As of February 2023, researchers are yet to find the exact cause of juvenile arthritis. No contemporary evidence suggests that JIA is caused by exposure to toxins, diet, or nutrient deficiency. Treatment for JIA solely focuses on managing symptoms and improving the quality of life. As gym regulars, we never cease to be amazed at the array of post-workout concoctions people consume in the changing room. We see everything from pills and powders to a rainbow of luminous drinks. Mostly it’s with one goal in mind, of course: to obtain muscles as close to the models endorsing these supplements as you possibly can. The global market for sports nutrition products is predicted to hit $45 billion (£33 billion) by 2022, an increase of about 60% from the 2016 value. Previously the domain of bodybuilders and elite athletes, amateur exercisers are becoming big supplement consumers thanks to the rising popularity of obstacle races, boot camps, and CrossFit all of which the product manufacturers target heavily. One particularly popular variety is products containing branched-chain amino acids (BCAAs). These seem to promise all the benefits of boosting muscle-building after exercise with none of the hassle of food. But do these products work – or are there better ways to help you get the most out of your gym membership? The protein in our muscles is constantly turning over, simultaneously being broken down and built up (“synthesized”). In young healthy adults, the rate at which these two processes occur is usually the same. But to grow muscles, the rate of synthesis has to be greater than the rate of breakdown. You can do this by combining a workout of lifting weights with consuming either a foodstuff rich in protein, such as whole milk, or the isolated amino acids derived from protein. Amino acids are the building blocks of protein and muscle. They come in 22 different varieties. A range of experiments has shown that three of these are particularly effective at activating the machinery responsible for increasing muscle protein synthesis. They are leucine, isoleucine, and valine – collectively known as the BCAAs. Supplement companies sell many products that contain just BCAAs, based on various findings about their importance, which mostly come from animal tests. Indeed, our own work suggests that leucine is particularly potent for activating the body’s protein synthesis machinery. Many people take this to mean that consuming isolated BCAAs will therefore generate a growth response as great as from an equivalent amount of food, without the additional calories or logistical problems of taking a meal to the gym. Yet recent work from our respective research teams suggests otherwise. We know for example that you can achieve a substantial increase in protein synthesis from 20g of whole egg protein, which is around three large eggs, or form 20g of whey protein. (Though to get the maximum possible response after a full body workout, our research on whey suggests it may be necessary to consume as much as 40g.) Under very similar lab conditions to the latter whey study, we gave a similar group of male subjects 5.6g of BCAAs following a session of lifting weights – the equivalent of 20g of whey. The resulting protein synthesis response was only about 22% – or about half what would be achieved with the equivalent dose of whey. What would the reason for this be? Various studies suggest that to achieve a full growth response, you need a sufficient supply of the whole complement of amino acids. This seems logical: think of it like building a brick wall where 20 different bricks are required to complete the wall. The BCAAs might act like the site foreman in bringing together all the men and machinery required to build the wall. But you still can’t build the wall without the rest of the bricks, no matter how much the foreman berates the bricklayers. In other words, following exercise you need whole protein sources that provide a high dose of BCAAs and sufficient amounts of all the other amino acids – for example, meat, dairy, or eggs. And not only are BCAAs less equipped to build the muscle wall but consuming them on their own potentially creates competition with other amino acids for absorption through the intestinal wall. If so, they could be making it harder for the body to gather up the other “bricks” essential to muscle building. Consuming protein-rich foods also contribute to the daily nutritional needs for a range of minerals and vitamins – something often lacking in supplements. Sticking with food will also help you avoid the risk of consuming a “banned” substance through supplementation. One study from 2007 tested 58 supplements and found that about a quarter was contaminated with banned substances. In short, don’t be taken in by the expensive luminous drinks in the changing rooms. From the reality of COIVD life to creating as pleasant an environment as possible, here are five ways the pandemic has influenced interior design trends in 2020. Had most homeowners, real estate agents, and interior designers been asked in January if they thought the open concept was here to stay, the answer would likely have been a resounding yes? But if you asked the same group in September, the answer might have been a little different. While open floor plans aren’t falling entirely out of fashion, they're no longer as practical and desirable as they once were. “Families still want big kitchens that open onto a family room—but home offices, outdoor spaces, and Zoom rooms. People have been looking for ways to create luxury upgrades to transform their homes into sanctuaries with amenities like meditation and massage rooms as well as secret gardens. But they want to limit the amount of money they spend on these projects. “During this time, many people need to stay on a budget, so it's a challenge to make a space beautiful and stay within a practical budget,” says Mrs. Soto. But the pandemic has truly had the biggest impact on those living in smaller spaces such as apartments. We’re looking to actually have defined spaces again, and some semblance of privacy and compartmentalization for things like working from home, exercising and people being home at the same time and taking calls. While some people have gone back to their regular offices at least part of the time, most of us are still working from home. A joint is where the ends of bones meet, such as the knee joint, shoulder joint, or the small joints in the fingers and toes.
Joint pain in children can be caused by a variety of things. But if a child's joints are swollen for 6 weeks in a row or longer, they may have chronic arthritis. Causes of chronic arthritis include juvenile arthritis, as well as other conditions like lupus or dermatomyositis. Juvenile arthritis is a long-lasting, chronic disease. It is the most common form of arthritis in children. In the United States, it affects nearly 300,000 children under the age of 18. There are several types of juvenile arthritis. Nearly all of them are different from rheumatoid arthritis in adults. This is why the term "juvenile rheumatoid arthritis (JRA)" is no longer widely used. Juvenile arthritis is a disease of inflammation. This means that inflammation in the body attacks healthy cells and tissues. Arthritis results from ongoing joint inflammation in four steps:
Oligoarticular means "few joints." In this type of juvenile arthritis, just a few joints are affected. About 50% of children with juvenile arthritis have the oligoarticular type. Girls younger than 8 years of age are more likely to develop it. In half of the children with oligoarticular juvenile arthritis, only one joint is involved, usually a knee or ankle. This is called monoarticular juvenile arthritis. In some cases, this arthritis may require only mild treatment. For some children, this arthritis affects four or fewer larger joints. Joints commonly affected include the knees or ankles. The involvement of fingers or toes is unusual. Oligoarticular juvenile arthritis may also cause eye inflammation. To prevent blindness, your child will need regular eye examinations from a doctor who specializes in eye diseases (ophthalmologist). Eye problems may continue into adulthood. About 30% of children with juvenile arthritis have the polyarticular type. This type of arthritis is more common in girls than in boys. Polyarticular juvenile arthritis affects five or more joints, including both larger joints (knees and ankles) and smaller joints (hands and feet). Often, both sides of the body are affected. Children with a certain marker, or antibody, in their blood, called IgM rheumatoid factor (RF), often have a more severe form of the disease. Antibodies are proteins in the blood usually used by the body to fight off infection through an immune response. In this form of arthritis, the IgM RF antibody attacks the body's own tissues. Doctors believe that this is the same type of arthritis as adult rheumatoid arthritis and can require more significant treatment. There are many treatment options for juvenile arthritis. The primary goal of all treatment options is to bring about the remission of arthritis. Remission means the child should not have any swelling or inflammation detected by exam or imaging. Treatment also focuses on preserving children's quality of life by making it possible for them to participate in play, sports, school, and social activities. In addition to treatment options, school administrators, social workers, and teachers can be important resources. They may be able to develop helpful lesson plans that teach classmates about juvenile arthritis. With treatment, children should be expected to be able to attend school and have excellent long-term outcomes. Although pain sometimes limits sports and physical activity, children with juvenile arthritis can often fully participate when symptoms are under control. The fact is public service encompasses all of these facets and then some. In this article, we explore the importance of public service and lay out four reasons why you may want to serve your community in your career. The technical definition of public service is a service provided by the government to the people in a specific jurisdiction. Services may be provided by the government itself, or they pay a private organization to provide them. For instance, a fire or police department is a government-run agency, but trash pickup provided by an independent contractor is a public service financed by the jurisdiction. The importance of public services – and public servants – cannot be overstated. In a Pew Research survey, more than 80% of respondents said they felt the government should play a major role in responding to terrorism and natural disasters, ensuring food and medicine are safe, and managing the immigration system. Maintaining the country’s infrastructure, protecting the environment, strengthening the economy, and ensuring access to education were also considered important government priorities. The same survey revealed that satisfaction with these efforts among Americans is relatively high. More than 70% of respondents said they thought the government was doing a “very good” or "somewhat good" job responding to natural disasters, setting standards for the workplace, keeping the country safe from terrorism, and managing food and medicine safety. Although satisfaction with certain efforts, such as the reduction of poverty, received a less favorable opinion, the ultimate takeaway is that public service matters. Beyond the public expectation that the government will provide certain services, public service matters for other reasons and represents a viable career option. Simply put, the array of public service career options is endless, especially as the lines between government, nonprofit agencies, and the private sector become increasingly blurred. Regardless of where your passion lies, public service presents opportunities. Often, when you ask someone why they entered public service their answer is “I wanted to make a difference.” What that looks like varies from person to person. For some, it’s rewriting public policy to effect change in a specific community, while for others it’s working in the field, providing life-saving assistance in emergencies. The concept of saving lives means different things to different people, but the desire to give everyone a fair shot at life underpins nearly every public service role. In an essay about public service for the Alliance of Innovation, Tracy Miller, the organization’s Florida regional director, wrote “You do not have to wonder as a citizen if you will have clean water to your home, or street lights that operate, or an ambulance when you dial 911. These have become givens in our lives and it is largely due to the stewards of our public dollars who balance the needs of the people they serve.” These services do often have challenges, and there are need for additional services to keep a community running safely and efficiently. Entering public service provides the opportunity to become a steward of public policy and contribute to the improvements that are vital to the quality of life. Community improvement isn’t only about improving fire department response times or maintaining clean playgrounds. Social change also matters, and public servants are increasingly becoming agents for social change, creating more equity in social conditions. A great deal of public service is focused on economics and creating more efficient and sustainable services and agencies, but there’s also an increasing focus on changing how policies and programs are administered to create more equitable conditions. From tackling issues related to racism and civil liberties to building support for citywide recycling programs, the opportunities for influencing social change through public service are diverse and plentiful. Finally, it can’t be overlooked that working in public service provides a significant source of personal fulfillment. Feeling as if you’re working for a purpose is important; after all, you will spend thousands of hours at your job every year, and work is often closely tied to your sense of identity and accomplishment. Choosing a career in public service can add to that sense of fulfillment. It can provide opportunities to act on your altruistic desires and the sense of accomplishment that comes from knowing you made a measurable difference in the lives of others. It makes me excited to share that there is actually a pastel of every colour in the rainbow! The pastel palette is absolutely appealing and adds a classy elegant quality to design of any space. Where colour lacks, detail thrives. That attention to detail is what designers look for in curating look and feel that work today or probably has been in vogue, time immemorial and will also carry on into the future. In design pastel means minimal or balancing colour, that are devoid of strong chromatic value or effect.
Shades such as pista or mint green, beige, peach, lavender, ivory, grey and shades of white give the impression to be without colour, but in many ways these hues mostly have undertones. These shades are often used an additional colour that act as softeners or play the equalizing shade when one or more shades of bold hues are used in an environment. Such shades also help achieve contrast and accentuate the brighter colours. For example, ivory might have an undertone of yellow or cream. White might be slightly ivory, yellow, bluish or peachy. Pastel colours can be used in design and decor in two primary ways -- either as a soft, pastel only, minimal look or as background colours for bold, sound accents. A Pastel colour theme in your home or any other space makes one feel calm, relaxed or settled and put you at comfort. While many could imagine pastel colours as mild or flat, there are many ways to create contrast, visual interest and still bring in light, soft colour for a synergetic look. When you want design with pastel shades, use different hues of the same colour, example shades of wheat or shades of grey for a classy, interesting and sophisticated look. Pick a lighter shade for the walls (vertical plane -as darker planes at eye level tend to look small) and compliment with darker upholstery. Choose a carpet or floor covering (horizontal plane) in a colour that complements the flooring but also is just a shade darker than the walls so the furniture elements stand out. Bind the space together with accentuating elements or decor that include few or all of the shades you have used. This enables all the elements to blend with in a room and give a harmonious look. When preferring to use pastel shades for all surfaces, where you choose to have an all pastel look, start the look with pastel walls. Choose the shade centred on what other shades you wish to use, how much natural light the room receives, what kind of artificial or planned lighting is required, the functional context, ambience you wish to create and the preference for lighter or darker shade walls. While opting darker shades for walls, the look ceases diminish volume and make it feel smaller, hence better to avoid darker shades for walls or balance with more illumination If you use grey, you can go either cool or warm with it and can contrast any colour against it. Also decide whether you prefer a warm or cool shade; the same goes for shades of white. When you choose a shade of beige for the wall or vertical plane, add volume with a brown or a woody colour furniture with upholstery from the darker shades of the beige family. Try to achieve gradation with your pastel colour family. Pick lighter shades of beige for horizontal plane or floor surface, this helps in achieving a look of better or bigger area. Use cushions in beige and metallic versions of beige-say mild copper to add interest. A pastel is the perfect backdrop for all the special details in your space, whether an art collection or pieces from world travels. Accessories and furnishings can speak volumes. A backdrop that is pastel helps those incidental elements stand out. Station art objects with touches of metallic finish to add some drama. There are many different versions of high-intensity workouts, including CrossFit and Tabata. But the latest trend is AMRAP, which stands for “as many reps (or rounds) as possible”. There’s little research looking specifically at the effects of AMRAP workouts. But based on what research shows about the effects of HIIT, it’s likely this short but adaptable form of exercise can provide similar health benefits.
AMRAP training is based on time. You work to complete as many repetitions or rounds of exercises as possible within a set time. For example, you could do ten press-ups, ten crunches, and ten jump squats, repeated in that order for as many rounds as possible until time expires. Or to make things simpler, you could perform a single exercise (such as burpees) for as many repetitions as possible for 60 seconds before taking a rest and repeating. There’s no specific time requirement, and workouts can be adjusted according to fitness levels. However, most AMRAP workouts tend to be between five and 30 minutes long, with no rest period after each exercise. Some Tabata workouts – which usually involve a person performing 20 seconds of intense exercise, followed by ten seconds of rest, repeated eight times – take as little as four minutes, but remarkably have been shown to improve a person’s aerobic fitness by as much as 18% following 24 exercise sessions over an eight-week training period. This is important because our level of fitness has a powerful protective effect against cardiovascular disease. In these studies, fitness is typically defined as the maximum amount of oxygen you can use during intense exercise – known as V̇O₂max. Though it’s seen a recent spike in popularity, AMRAP training isn’t new. This form of training has been used in high-intensity circuit training for decades – most recently popularized by CrossFit. Although relatively little research has investigated the effects of AMRAP training specifically, strong evidence shows HIIT can improve blood pressure, blood sugar control, and lower body fat. Also, improvements in aerobic fitness following HIIT exercise are similar – or even greater than – those seen after typical endurance training, such as running or cycling, despite a significantly lower exercise volume. Aerobic fitness is important for our health and might even be more important than the total amount of exercise that we do. The key reason HIIT seems to be so effective is because it rapidly uses muscle glycogen (stored carbohydrates) through a process known as glycogenolysis Research shows that during HIIT performed on a cycling machine, it’s the rate of glycogenolysis that acts as a “signal” to the body to increase fitness, which in turn leads to improvements in health and performance. In short, the intensity of exercise is important. Although it’s uncertain if a similar mechanism happens following AMRAP workouts, it’s reasonable to assume they might be similar, especially if the activity is performed at a high intensity. Indeed, CrossFit has been shown to improve aerobic fitness, anaerobic capacity, and body composition (lowering body fat and increasing muscle) in people with varying fitness levels. However, there are drawbacks to AMRAP-type workouts. The intensity of the exercise could make it difficult or unsuitable for many, particularly those who aren’t active or have long-term medical conditions. Also, high-intensity AMRAP can be very physically challenging so it’s likely to feel unpleasant, which might make some people less likely to stick with this type of exercise. Tabata-type workouts are shown to be less enjoyable to participants, and other approaches to HIIT cause greater displeasure compared with moderate-intensity continuous cycling. Just as with any workout, the benefits of AMRAP will only be seen if a person continues doing them. But our research has found that reduced-exertion approaches to HIIT can reduce strong feelings of displeasure during workouts. This type of HIIT decreases the total time spent completing the high-intensity exercise, making it more tolerable. However, it’s uncertain if balancing volume and intensity during exercise brings the same positive health changes following HIIT. Higher-intensity exercise also raises concerns about injury risk. A recent review found the number of injuries sustained during CrossFit was quite high (between 19% and 74% of participants). Most injuries were to the shoulder or lower back. The challenge of working to a time limit during AMRAP could lead to poor technique, which may increase injury risk – particularly in those who are inexperienced. Regular high-intensity workouts could also increase the chances of developing rhabdomyolysis, a rare but dangerous condition where muscle damage increases calcium levels, although the risk remains low. AMRAP training is highly versatile and can be adapted for a wide range of fitness levels and environments. For example, it can be done at home by doing only body-weight exercises. The intensity of the activity can also be modified to make it more manageable, but this may not lead to significant fitness increases. It may be a cliché but it’s true: the best exercise is the exercise you will do. AMRAP will be effective for some but it might not work for everyone. Good exercise techniques, progressive overload, and sticking to a routine are important for success. |
